Williston, Chittenden County'; Census independently confirms '45 KRUPP DR, WILLISTON, VT, 05495'). Recorded here under its true municipality (Williston) per this wave's geocoding-verification requirement rather than the aggregator's market-name label. Only a single third-party directory source was found for this facility's existence at all (no operator-page confirmation succeeded), and given the source's demonstrated city error, confidence is 'reported' rather than 'confirmed.'
